Does a narcissist ever feel guilty about the way they hurt you?

If they have to see you afterwards then yes. The shame and guilt he would feel would be palatable. They definitely dont feel it in the moment of the act or when they are discarding. But it hits them later if they have hoovered you in. I wouldnt even be the one to bring up the past and the things he did he would be. When he would bring it up he couldnt even look me in the face and he being an npd he of course tried shifting the blame or making excuses for the behavior but he most definitely felt guilty. of course he doesnt like how that makes him feel so once he was no longer in my presence he started a text argument acting talking like an asshole. Then he would be fine a couple days later loke none of it ever took place. People always think npd dont have any feelings they actual are very sensitive people the problem is in how they react to those feelings they have. Nothing is easy with them you have to have a deep awareness of them and how they think, study their behavior to learn their triggers. It takes time, patience, a ton of forgiveness, and knowing your own bounderies with what your willing to sacrifice to be there for them. Dont have any expectations for yourself because they dont know how to reciprocate it just isnt in their mindset. They are very superficial because they are very uncomfortable with anything else.
